Initial Comments:
An offsite follow-up survey conducted on January 26, 2024 found that Keystone Hospice had corrected deficiencies cited under 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care. The deficiencies were cited as a result of a Medicare Re-Certification and State Re-Licensure Survey completed on November 17, 2023.
